Concussion Information Presentation to Team (and parents?)

 

  • A brief and complete definition of what constitutes a concussion.
  • Examples of common mechanisms of injury everyone should be familiar with.
  • A simple list of the common signs and symptoms of a concussion.
  • A thorough explanation regarding what should be done in the event of a suspected head injury.
  • If a head injury is expected, our step-by-step approach to getting the injured athlete the proper care required to ensure a safe return to activity.

 
 

Concussion Information Handouts

 

  • Athlete information handout explaining how to recognize signs and symptoms of a concussion and what to do if a concussion is suspected.
  • Parent information handout illustrating how to recognize signs and symptoms of concussion and what to do if a concussion is suspected.
  • School staff information handout detailing how to recognize mechanisms of injury and signs and symptoms of concussion as well as what to do if a concussion is suspected in one of your student athletes.

 
 

Health History Form

 

  • To be completed by Athlete/Parent/Guardian. This form is necessary to help our staff to pre-screen athletes who may have potential predisposing factors that could contribute to concussion susceptibility in the future.

 
 

Informed Consent Authorization

 

  • To be completed by Athlete/Parent/Guardian. This form allows us to complete the pre-screening program with the athletes or parent/guardian’s permission. It also allows us to forward necessary information to our concussion specialist physician should a head injury be suspected.

 
 

ImPACT Baseline Test

 

  • ImPACT (Immediate Post-Concussion Assessment and Cognitive Testing) is the first, most-widely used, and most scientifically validated computerized concussion evaluation system.
  • This test is to be completed on all players. This online evaluation takes approximately 30 minutes to complete and is usually done in quiet room with adequate space for up to twenty athletes at a time.
  • Given the inherent difficulties in concussion management, it is important to manage concussions on an individualized basis and to implement baseline testing and/or post-injury neurocognitive testing. This type of concussion assessment can help to objectively evaluate the concussed athlete's post-injury condition and track recovery for safe return to play, thus preventing the cumulative effects of concussion.
  • All information gathered is stored securely with ImPACT USA.
  • Access to the information available to Panno Medical Inc.

 
 

SCAT 2 Baseline Test

 

  • Upon review of the medical history questionnaire, if any athlete has a history of concussions we will complete a SCAT 2 pre-season baseline assessment as well as the ImPACT pre-season test.
  • SCAT 2 (Sport Concussion Assessment Tool – Version 2) is a recognized, standard method of evaluating athletes for concussion.
